Job Title: Physical Therapist for Home Health Visits

We are seeking a skilled Physical Therapist to join our Home Healthcare Team at Physicians Choice Home Health. As a Physical Therapist, you will be responsible for providing high-quality home health visits to our patients in the comfort of their own homes.

Job Summary:

The Physical Therapist will assess and evaluate patients' therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care. You will also direct physical therapy treatment, instruct patients and families/caregivers in the use and care of therapeutic appliances, and determine priority needs for physical therapy.

Responsibilities:
  • Assess and evaluate patients' therapeutic, rehabilitative, and functional status
  • Participate in the development of the total plan of care
  • Direct physical therapy treatment
  • Instruct patients and families/caregivers in the use and care of therapeutic appliances
  • Determine priority needs for physical therapy
  • Report to physician patient's reaction to treatment or changes in condition
  • Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy
  • Identify patient and family/caregiver needs for other home health services and refer as necessary
  • Prepare and submit clinical and progress summaries based on the attainment of goals
  • Participate in discharge planning for patient
Requirements:
  • Possesses a degree from a Bachelors or Masters' program in physical therapy approved by an accredited organization
  • Licensed to practice as a physical therapist within the state, if applicable
  • Two years of appropriate experience as a physical therapist
  • Demonstrates good verbal and written communication and organizational skills
  • Possesses and maintains current CPR certification
  • Ability to be able to travel to different patients' homes
  • Discretion in dealing with sensitive and confidential information

Physician Choice Home Health currently pays PTs $60-110/visit and $40/hour for in-services, trainings, and orientations. Per visit mileage reimbursement is offered for W2 employees.
