Certified Home Health Aide for Home Visits

We are seeking a compassionate and dedicated Certified Home Health Aide to join our Home Healthcare Team at Physicians Choice Home Health. As a Home Health Aide, you will provide quality home health visits to our patients in the comfort of their own homes, working under the supervision of a registered nurse or therapist.
Key Responsibilities- Provide hands-on personal care, including bathing, back rubs, oral hygiene, and skin care to prevent breakdown.
- Assist patients with dressing and undressing, toileting activities, and keeping their living area clean and orderly.
- Plan and prepare nutritious meals, assist with feeding, and take and record vital signs as ordered.
- Assist with ambulation and exercise, perform range of motion and other simple procedures as an extension of therapy services, and assist with medication administration.
- Meet safety needs of patients, use equipment safely and properly, and report on patient condition and significant changes to the assigned nurse.
- Meets training requirements and successfully completed a training and competency evaluation program that meets §484.80 (b) and (c).
- Shows proof of registration on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau, preferred.
- At least 18 years of age, ability to read and follow written instructions, and document care given.
- Self-directing with the ability to work with little direct supervision, empathy for the needs of the ill, injured, frail, and impaired, and current CPR certification.
