Physical Therapist for Home Health Visits

We are seeking a skilled Physical Therapist to join our Home Health Team at Physicians Choice Home Health. As a Physical Therapist, you will be responsible for providing high-quality home health visits to our patients, promoting their independence and well-being in the comfort of their own homes.
Key Responsibilities- Assess and evaluate patients' physical therapy needs, developing personalized plans of care to achieve optimal outcomes.
- Provide direct physical therapy services, including exercises, modalities, and education, to patients in their homes.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Conduct home safety assessments and provide education on fall prevention and home modifications.
- Document patient progress and outcomes, ensuring accurate and timely reporting.
- Master's or Doctoral degree in Physical Therapy from an accredited program.
- Current licensure to practice as a Physical Therapist in the state of employment.
- Minimum 2 years of experience in home health or a related setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to travel to patients' homes, with reliable transportation and a valid driver's license.
