Job Overview

Objective

The Child Welfare Case Coordinator plays a vital role in delivering comprehensive services across various programs, including Foster Care, Independent Living, Family Support, Reunification, and Diversion. The overarching aim is to instill hope and facilitate positive change in individuals and families. The specific services, responsibilities, and clientele may vary based on the assigned program(s).

Key Responsibilities

  1. Delivers both in-home and external case management services to a designated caseload, adhering to supervisory guidelines and regulatory limits.
  2. Ensures effective coordination, provision, and documentation of necessary services to clients, collaborating closely with external providers and agency personnel.
  3. Establishes trust and positive connections with youth and their families.
  4. Conducts and records face-to-face and/or indirect interactions with assigned youth and families at least weekly, in line with program standards.
  5. Collaborates with youth, caregivers, therapists, educational institutions, and other team members to evaluate needs and safety, striving for optimal outcomes for clients.
  6. Regularly consults with the referring agency worker throughout the service duration.
  7. Maintains professional relationships with youth, families, social workers, educational staff, therapists, foster caregivers, and other community and team members.
  8. Completes program-specific assessments with youth and families.
  9. Participates in crafting service plans, prevention strategies, and aftercare plans alongside youth, families, and therapists when applicable.
  10. Assists clients in recognizing and altering negative or high-risk behavioral patterns.
  11. Encourages clients to practice and celebrate incremental changes.
  12. Connects families with community resources to enhance overall family functioning.
  13. Remains available according to program-specific on-call protocols.
  14. Responds to behavioral and emotional crises of clients, providing consultation, coaching, and/or in-person intervention as necessary.
  15. Develops a transition/aftercare plan prior to case closure and conducts follow-ups as required.
  16. Documents services rendered and progress achieved in compliance with program-specific regulatory standards.
  17. Completes intake and discharge documentation as mandated by the program.
  18. Attends all required training sessions and individual/team consultations.
  19. Performs additional duties as assigned by the program supervisor.

Education and Experience

A Bachelor’s degree (B.A.) from an accredited four-year college or university in a behavioral science field, along with a minimum of one year of full-time experience working directly with individuals facing behavioral health challenges. A Master’s degree in behavioral science may substitute for the required experience.
