Job Summary

The Patient Advocate is a key member of the Nexus Health Systems team, responsible for ensuring a positive and supportive experience for patients and their families during their stay at our hospital.

Key Responsibilities
  • Welcoming Patients and Families: Provide a secure and comforting welcome to patients and their support persons upon admission.
  • Pre-Admission Outreach: Contact families and/or patients one day before admission to introduce the role and review the admission process.
  • Post-Admission Support: Follow up with patients and families during the first week of admission and weekly thereafter to ensure their needs are being met.
  • Patient Education and Community Engagement: Educate patients, families, and community members about Nexus Health Systems through private discussions and community events.
  • Leadership and Collaboration: Lead the Patient Experience Committee and collaborate with leadership and treatment team members to ensure positive or negative feedback from patients, families, or other stakeholders is addressed.
  • Family Support and Resources: Organize family support groups and maintain a Family Resource handbook to provide patients and families with necessary information and support.
  • Process Improvement: Make recommendations for improvements in operational processes to enhance the patient experience at Nexus Health Systems.
Requirements
  • Education: High school diploma or equivalent required; bachelor's degree in a healthcare field preferred.
  • Experience: 3-5 years of direct patient healthcare experience; experience in case management and/or advocacy.
  • Language Skills: Bilingual speaker preferred.
