Position Overview:
We are actively looking for skilled individuals to fill a part-time role as a Registered Nurse (RN) Care Coordinator.
Job Responsibilities:
This position is focused on outpatient care and requires a self-sufficient and seasoned RN. The RN Care Coordinator plays a crucial role within the interdisciplinary care team, responsible for managing the admission process and ensuring suitable care for patients and their families. Key responsibilities include, but are not limited to:
  • Patient admissions
  • Comprehensive assessments
  • Patient discharges
  • Medical pronouncements
Required Qualifications:
  • Valid RN license in the State of Texas
  • Minimum of 1 year of RN experience preferred
  • Background in Hospice or Home Health care preferred
  • Strong written and verbal communication skills
  • Ability to travel with reliable transportation
  • Clean background check, drug screening, and driving record
