Position Overview:

The Youth Transition Clinician plays a crucial role in delivering therapeutic services to adolescents and their families following their discharge from a residential program.

This position encompasses a range of responsibilities, including providing individual, group, and family therapy, as well as effectively communicating the clinical requirements of youth to various stakeholders such as collaborative agencies, families, and legal representatives.

The clinician serves as the primary point of contact for ongoing treatment as youth reintegrate into their community.

The goal is to support youth and their families in adjusting to their return to the community.


Key Responsibilities:


Act as the clinical lead in delivering and coordinating necessary therapeutic services for youth and families, which may involve travel to their homes or community settings.

Engage consistently with youth, families, and staff in a professional manner that reflects positively on the organization and promotes effective outcomes.

Maintain a constructive rapport and professional relationships with families and county representatives, including caseworkers and legal guardians.

Understand and adhere to all approved policies, protocols, and procedures of the organization.

Collaborate with the immediate supervisor to establish and achieve annual performance improvement goals.

Ensure timely and accurate record-keeping and reporting to assist youth and demonstrate compliance with job expectations.

Exhibit proficiency in grammar, spelling, and syntax in all correspondence.

Meet established deadlines for paperwork and attendance at meetings and events.


Perform additional duties as assigned by the Aftercare Coordinator, which may include:

  • Providing individual, group, and family therapy within community settings.
  • Maintaining regular contact with family members, service providers, and county representatives.
  • Completing clinical documentation and assessments, including treatment plans and risk evaluations.
  • Meeting the minimum required billable hours per week.
Qualifications:

Master's Degree or licensure in a mental health field (LMSW, LMFT, LMHC, etc.) is required.

Experience and familiarity with mental health environments are essential.

Knowledge of community resources and support systems is advantageous.

Ability to work independently while effectively collaborating with a multidisciplinary team is necessary.

A valid Driver's License and proof of automobile insurance are required.
