Key Responsibilities
  • Identify and connect individuals with community resources and services to support their behavioral health needs.
  • Develop and implement aftercare plans to ensure seamless transitions from hospital to community settings.
  • Provide ongoing support and follow-up to individuals to address gaps in care and obstacles to accessing services.
  • Collaborate with healthcare providers, community organizations, and government agencies to foster a comprehensive approach to behavioral health care.
  • Manage and document aftercare follow-up for high-risk individuals, ensuring timely and effective interventions.
Requirements
  • Master's degree in a behavioral health-related field or equivalent experience.
  • Active, unrestricted license to practice in the State of New York.
  • 2+ years of experience in a related mental health environment.
  • Proficient computer skills and working knowledge of Excel spreadsheets.
  • Access to reliable transportation and a dedicated workspace.
Preferred Qualifications
  • Experience working in medical or behavioral settings, including hospitals, managed care organizations, or joint medical/behavioral outpatient practices.
  • Dual diagnosis experience with mental health and substance abuse.
  • Case management experience.
  • Field-based experience.
  • Experience working with low-income populations, the aged, blind, or disabled, or in community health settings.
