We have an exciting opportunity for a highly motivated Patient Transition Liaison to join our team at Multicare Health System. In this role, you will play a crucial part in ensuring seamless transitions of care for our patients, bridging the gap between healthcare settings and fostering a culture of collaboration.

**About the Position:**

As a Patient Transition Liaison, you will work closely with patients, families, healthcare professionals, and community agencies to coordinate care and promote healthy transitions. Your key responsibilities will include assessing patient needs, developing personalized care plans, and collaborating with interdisciplinary teams to deliver high-quality, patient-centered care.

**Primary Responsibilities:**

  • Coordinate and communicate with patients, families, physicians, and other healthcare professionals to facilitate smooth transitions of care
  • Assess individual patient needs, develop personalized care plans, and implement strategies to achieve optimal health outcomes
  • Collaborate with multidisciplinary teams to identify and address complex patient needs, promoting a culture of care coordination and collaboration
  • Document patient interactions, care plans, and outcomes in accordance with organizational standards

**Requirements and Qualifications:**

  • Master's degree in Social Work or related field
  • Licensed in the state of Washington as Social Worker Associate Independent Clinical (LSWAIC)
  • Minimum 3 years' experience in healthcare, preferably in a hospital or clinical setting
  • Strong understanding of healthcare systems, policies, and procedures
  • Excellent communication, interpersonal, and problem-solving skills
