Job Summary

We are seeking a highly skilled Registered Nurse to join our team at Prisma Health. As an Advanced Patient Care Specialist, you will provide clinical direction for all aspects of patient care in an acute inpatient setting, fostering a patient-centered approach.

Responsibilities

  • Develop and implement individualized plans of care based on the assessed needs of patients, collaborating with healthcare teams to ensure effective communication and coordination.
  • Monitor patient progress, updating plans as necessary to ensure continuity of care and a smooth transition post-discharge.
  • Evaluate opportunities for quality improvement and incorporate recommendations into the plan of care as appropriate.
  • Teach patients and families about their health conditions, medications, and treatment options, promoting self-advocacy and empowerment.
  • Serve as a patient advocate, coordinating care and communicating with providers and other healthcare professionals as needed.

Requirements

  • Degree in Nursing (Associate or Bachelor's degree preferred)
  • One year of experience as a Registered Nurse (or successful completion of the Prisma Health Nurse Residency Program or comparable residency)
  • Licensed to practice as a Registered Nurse in South Carolina

Estimated Salary Range: $65,000 - $85,000 per annum, depending on qualifications and experience.
