We are seeking a dynamic and empathetic Healthcare Liaison Specialist to join our team at MultiCare Health System. As a liaison between patients, families, and healthcare providers, you will play a critical role in ensuring smooth transitions of care and optimal patient outcomes.

Estimated Salary: $93,121 - $125,244 per annum

About the Job
  • You will serve as a single point of contact for patients and families, coordinating care and addressing concerns in a timely and compassionate manner.
  • Collaborate with interdisciplinary teams to develop and implement personalized care plans that meet the unique needs of each patient.
  • Foster strong relationships with patients, families, and healthcare providers to ensure seamless communication and optimal patient outcomes.

Requirements:

  • A Master's degree in Social Work or a related field is required.
  • Licensed Independent Clinical Social Worker (LICSW) or Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain LICSW within four years.
  • Minimum three years of experience in a healthcare setting, preferably in a hospital or equivalent environment.
