As a key member of our thoracic surgery team, you will play a critical role in coordinating referrals, providing education, and facilitating smooth transitions for patients undergoing lung cancer screening.

About the Job Description:

As a Nurse Navigator, Surgery, you will be responsible for:

  • C Coordinate referrals and communicate with patients, families, and healthcare providers to ensure seamless transitions
  • Educate patients on their diagnosis, treatment options, and follow-up care requirements
  • Work collaboratively with multidisciplinary teams to develop comprehensive care plans and reduce readmissions
  • Provide support and resources to patients and families affected by lung cancer
  • Foster strong relationships with referring physicians, radiology staff, and other healthcare providers to optimize patient care
