Patient Care Coordinator

About the Role:
CenterWell is seeking a skilled Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a critical role in developing and maintaining relationships with healthcare providers, hospitals, and skilled nursing facilities to ensure seamless care transitions for our patients.
Key Responsibilities:
- Develop and maintain relationships with key stakeholders in the healthcare industry, including hospitals, skilled nursing facilities, and assisted living facilities.
- Conduct facility visits to assess patient needs and provide education on our services.
- Collaborate with our clinical team to determine patient eligibility for our services.
- Ensure effective communication and collaboration with program staff and other field sales resources.
- Process physician orders in a timely manner.
Requirements:
- RN/LPN/LVN license.
- Strong knowledge of governmental regulations and Medicare eligibility requirements.
- Comprehensive understanding of potential care plan needs for patients and coordination of necessary resources.
- Excellent customer service, account development capabilities, organization, time management, problem-solving, communication, and selling skills.
Preferred Qualifications:
- Bachelor's of Science in Nursing.
- A minimum of three years clinical experience.
