This position requires a commitment of 32 hours per week, including 12-hour shifts on weekends and an additional 8-hour shift on either Friday or Monday.
As a Patient Care Management Coordinator, your responsibilities will include:
+ Establishing and nurturing relationships with key healthcare facilities, including hospitals and skilled nursing centers, to provide continuous updates on available services.
+ Conducting facility visits at the request of physicians to assist the clinical team in assessing patient eligibility.
+ Facilitating effective communication and collaboration with program staff and other sales resources through regular meetings. Actively participating in business development discussions, presenting relevant data and insights regarding account changes and referral sources.
+ Supporting the program in the prompt processing of physician orders as needed.
Utilize your expertise to create a positive impact
Essential Qualifications:
+ Valid RN/LPN/LVN license.
+ In-depth knowledge of regulatory standards, Medicare eligibility criteria, and a comprehensive understanding of patient care planning and resource coordination.
+ Strong customer service skills, account management abilities, organizational skills, time management, problem-solving capabilities, effective communication, and sales proficiency.
Preferred Qualifications:
+ A Bachelor’s degree in Nursing is preferred.
+ At least three years of clinical experience is preferred.
Work Schedule:
40 hours per week
Compensation Range:
The salary range provided reflects a good faith estimate of starting base pay for full-time employment. Actual compensation may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications. The estimated range is $57,700 - $79,500 annually.
