We are seeking a skilled Patient Care Coordinator to join our team at CenterWell Home Health. Key Responsibilities:
- Develop and Maintain Relationships: Establish and maintain strong relationships with key healthcare providers, hospitals, skilled nursing facilities, and assisted living facilities to ensure effective discharge planning and coordination of care.
- Conduct Facility Visits: Conduct regular facility visits to assess patient needs, provide education, and facilitate the development of comprehensive care plans.
- Ensure Effective Communication: Ensure timely and effective communication with program staff, field sales resources, and other stakeholders to ensure seamless coordination of care.
- Process Physician Orders: Assist in the timely processing of physician orders and ensure that all necessary documentation is completed accurately and efficiently.
Requirements:
- Education and Experience: RN/LPN/LVN license, with a minimum of three years of clinical experience preferred.
- Skills and Qualifications: Strong knowledge of governmental regulations, Medicare eligibility requirements, and comprehensive understanding of potential care plan needs for patients. Excellent customer service, account development capabilities, organization, time management, problem-solving, communication, and selling skills.
Preferred Qualifications:
- Education: Bachelor's of Science in Nursing preferred.
- Experience: A minimum of three years of clinical experience preferred.
