Position Overview

The LPN Care Coordination Specialist plays a vital role within Centra Health, functioning as part of a Nurse Resource Pool to deliver exceptional nursing services across various medical practice locations.

This role operates under the guidelines set forth by the Virginia State Board of Nursing and under the guidance of a registered nurse.

The licensed practical nurse (LPN) is responsible for organizing and delivering patient care in alignment with Centra's Professional Practice Model.

The LPN works collaboratively with healthcare providers and the interdisciplinary team to ensure the delivery of high-quality patient care. The LPN is instrumental in coordinating care processes to achieve optimal patient outcomes, fostering a culture that embodies Centra's mission, vision, and values.

The practice of the LPN adheres to the ethical standards outlined by the American Nurses Association (ANA), the Nurses Practice Act of Virginia, and the ANA Principles of Nurse Staffing.



Key Responsibilities

• Travels to various Centra Health medical practice sites to provide nursing services as required

• Assists in conducting physical examinations and gathering health histories

• Creates a supportive environment by offering emotional, psychological, and spiritual assistance to patients and their families

• Engages in health promotion, counseling, and education for patients and caregivers, establishing rapport to identify care needs

• Administers medications, wound care, and personal health interventions as directed by a healthcare provider

• Maintains precise and comprehensive patient records using an electronic medical records (EMR) system

• Collects and shares patient information to collaborate effectively with providers and interdisciplinary team members

• Ensures a safe and clean working environment by adhering to established policies, procedures, and regulations

• Complies with CMG Quality Metrics as mandated by regulatory bodies

• Upholds patient confidentiality and protects operations by following HIPAA regulations

• May undertake additional duties as assigned or requested, with the understanding that job specifications may be modified or updated as necessary

Qualifications

Education Requirements:
Graduate of an accredited Nursing or LPN Program

Experience Requirements:
Minimum of two (2) years of experience as an LPN. Must maintain a professional portfolio that includes current licensure, annual Virginia Board of Nursing Education Requirements, and completion of Centra's mandatory training

Certifications and Licensure:

Must hold a current, active license as a practical nurse in Virginia or possess a current multistate licensure privilege. BLS certification is required within the first six (6) months of employment and must be maintained thereafter.

Driving is a critical function of this role; a valid Virginia State Driver's License with a good standing DMV record is required.

When operating a Centra-owned vehicle, Centra serves as the primary insurer. For privately-owned vehicles used for work, personal insurance is the primary coverage, with Centra insurance providing supplemental coverage for catastrophic events. Centra will review DMV records annually and verify personal insurance as needed.
