Overview

Position Summary:

The Patient Safety Improvement Specialist is responsible for overseeing daily initiatives aimed at enhancing patient safety across designated facilities.

Key Responsibilities:

  • Effectively leads patient safety initiatives and mentors colleagues in fostering a culture of safety, focusing on process enhancement and achieving zero harm.
  • Conducts thorough investigations and ensures resolution of all patient safety incidents.
  • Keeps detailed records of patient-related errors and analyzes this data to uncover root causes. Proposes modifications, policies, or programs to avert future incidents.
  • Supports the coordination of the annual review and execution of the organization’s Patient Safety Plan, along with the development of the Quality Assessment & Performance Improvement Plan.
  • Generates quality reports and presentations in preparation for quality committee sessions.
  • Facilitates department and facility Patient Safety meetings, assesses trends, identifies areas for enhancement, formulates action plans, evaluates the success of these plans, and maintains records and follow-up metrics for each meeting and activity; continuously reassesses strategies to optimize outcomes.
  • Assists leadership in establishing key performance indicators and creating evaluation tools to measure quality advancements, and coordinates efforts for patient safety culture assessments.
  • Formulates improvement strategies that incorporate leading practice research and disseminates implementation outcomes throughout the organization, providing education and training on patient safety and performance enhancement tools.
  • Engages in and/or coordinates projects, initiatives, and collaborations across multiple facilities or business units.
  • Consults with and trains team members to adopt monitoring and measurement tools, as well as data analysis methodologies.
  • Exemplifies best practices and advocates for safety and high reliability within the organization.
  • Identifies and mitigates risks related to patient safety.
  • Enhances awareness and practices of patient safety among healthcare providers and team members.
  • Conducts root cause analyses in response to unforeseen events that could lead to serious physical harm, monitoring the effectiveness of implemented actions.
  • Contributes to other projects and initiatives as required.

Qualifications:

A Bachelor's degree is required; a Master's degree is preferred.

Clinical licensure such as RN is preferred.

4-6 years of experience in healthcare or a closely related field, with 2 to 4 years of leadership experience preferred.

Familiarity with Lean/Improvement Concepts is preferred.

Intermediate to advanced proficiency in Microsoft Office applications is preferred.

Understanding of Patient Safety Principles and Just Culture is preferred.

Certification in Healthcare Quality, Patient Safety, or a related accreditation entity is preferred.
