Overview

Position Summary:
The Patient Safety Improvement Specialist is responsible for overseeing daily initiatives aimed at enhancing patient safety across designated facilities.

Key Responsibilities:


Effectively leads patient safety initiatives and mentors colleagues in fostering a culture of safety, focusing on process enhancement and zero harm principles.

Investigates and addresses all incidents related to patient safety, maintaining comprehensive records of errors and assessing this data to uncover root causes. Proposes modifications, policies, or programs designed to avert future incidents.

Supports the coordination of the annual review and execution of the organization's Patient Safety Strategy and the formulation of the Quality Assessment & Performance Improvement Plan.

Generates quality reports and presentations in preparation for quality oversight meetings.

Facilitates department and facility-level Patient Safety meetings, analyzes patterns, identifies areas for enhancement, formulates action plans, measures the effectiveness of these plans, and documents follow-up metrics for each meeting and initiative; continuously reassesses strategies to optimize outcomes.

Assists management in establishing key performance indicators and developing evaluation tools to assess quality advancements, coordinates assessments of patient safety culture.

Crafts improvement strategies that incorporate leading practice research and disseminates implementation outcomes throughout the organization, provides training and education on patient safety and performance enhancement tools.

Engages in and/or leads multi-facility/business unit and/or Market-wide projects, initiatives, and collaborations.
Consults with and instructs team members to apply monitoring and evaluation tools and data analysis methodologies.
Exemplifies best practices and advocates for safety and high reliability within the organization.
Identifies and mitigates risks associated with patient safety.
Enhances awareness and practices of patient safety among healthcare providers and staff.
Conducts root cause analyses in response to unforeseen events that may result in serious physical harm. Monitors implemented actions to assess their effectiveness.
Supports additional projects and initiatives as needed.

Qualifications:
A Bachelor's degree is required; a Master's degree is preferred.
Registered Nurse or equivalent clinical licensure is preferred.
4-6 years of experience in healthcare or a closely related field, with 2 to 4 years of leadership experience preferred.
Familiarity with Lean/Improvement Concepts is preferred.

Intermediate to advanced proficiency in computer skills, particularly in Microsoft Office applications, is preferred.

Knowledge of Patient Safety Principles and Just Culture is preferred.
Certification in Healthcare Quality, Patient Safety, or a related accreditation body is preferred.
