Overview

Position Summary:
Responsible for overseeing ongoing enhancements in patient safety across designated facilities.

Key Responsibilities:


Effectively leads initiatives aimed at improving patient safety and mentors colleagues in fostering a culture of safety, focusing on process innovation and zero harm principles.

Investigates and resolves all incidents related to patient safety. Maintains comprehensive records of patient-related errors and analyzes this data to uncover root causes. Proposes modifications, policies, or programs to avert future incidents.

Assists in the annual review and execution of the organization's Patient Safety Strategy and the development of the Quality Assessment & Performance Improvement Plan.

Prepares quality-related reports and presentations for quality committee sessions.

Facilitates departmental and facility-level Patient Safety meetings, assesses trends, identifies areas for enhancement, formulates action plans, evaluates the effectiveness of these plans, and documents follow-up metrics for each meeting and initiative; continually reassesses strategies to optimize outcomes.

Collaborates with leadership to establish key performance indicators and create evaluation tools for quality enhancements, coordinates assessments of patient safety culture.

Formulates improvement strategies that incorporate leading practice research and disseminates implementation outcomes throughout the organization, providing education and training on patient safety and performance enhancement tools.

Engages in and/or coordinates multi-facility or market-wide projects, initiatives, and collaborations.
Consults with and trains staff to implement monitoring and measurement tools along with data analysis methodologies.
Exemplifies best practices and advocates for safety and high reliability within the organization.
Identifies and addresses patient safety vulnerabilities.
Enhances awareness and practices of patient safety among clinical staff and colleagues.
Conducts root cause analyses for unexpected events that could lead to serious physical harm. Monitors remedial actions to assess their effectiveness.
Supports additional projects and initiatives as required.

Qualifications:
Bachelor's degree is mandatory; Master's degree is preferred.
RN or equivalent clinical licensure is preferred.
4-6 years of experience in healthcare or a closely related field, with 2 to 4 years of leadership experience preferred.
Familiarity with Lean/Improvement Concepts is preferred.

Intermediate to advanced proficiency in computer skills, particularly in Microsoft Office applications, is preferred.

Understanding of Patient Safety Principles and Just Culture is preferred.
Certification in Healthcare Quality, Patient Safety, or a related accreditation body is preferred.
