CommuniCare Health Services Corporate is seeking a skilled Patient Assessment Coordinator to identify patients for potential admission into our facilities and maintain positive relationships with current referral sources.

The ideal candidate will have an RN or LPN license, a degree in Social Work, or other medical credentials. They will be responsible for facilitating admissions by coordinating transmission of clinical and benefit information from referral sources back to the SNF admissions teams, administrations, and third party payers.

Key Responsibilities:
  • Performing clinical assessments
  • Providing excellent customer service
  • Coordinating with referral sources and SNF admissions teams
  • Maintaining accurate and timely clinical assessments

The Patient Assessment Coordinator will work closely with all departments in the facility to achieve these objectives. They will be responsible for providing comprehensive clinical assessments and working proactively with all departments to ensure seamless admissions processes.

Requirements:
  • Current, valid and active license to practice as a Registered Nurse or Licensed Practical Nurse in the state of Ohio, OR Social Work Degree, OR other medical credential
  • Three (3) years treatment experience in a Long Term Care setting preferred
  • Previous clinical screening experience preferred
  • Recent healthcare experience in Long Term Care preferred
  • Knowledge of local medical community preferred
