Nurse Practitioner Home Risk Assessment Specialist

As a Home Risk Assessment Specialist, you will be part of a team that breaks through traditional barriers to care by meeting patients where they are, in the comfort of their own homes.
We are seeking a skilled and compassionate Nurse Practitioner to join our team of home-based healthcare professionals. In this role, you will conduct comprehensive health assessments, reviewing a patient's current health, medical history, medication adherence, social environment, and other risks. This unparalleled insight into a patient's overall health and well-being enables us to identify potential care gaps and provide targeted interventions to improve quality of care.

- Conduct comprehensive health assessments, including medical history, diagnosis, and treatment, health education, physician referrals, and case management referrals.
- Work collaboratively with physicians, case managers, social workers, family members, and other healthcare professionals to ensure seamless care coordination.
- Provide patient education and follow-up care, collaborating with Primary Care Physicians as needed.
- Deliver services in various settings, including home visits, skilled facility visits, and telehealth visits.
- Master's degree in Nursing or equivalent experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with electronic health/medical record software.
- Excellent verbal and written communication skills, with the ability to interact effectively with patients, clients, and colleagues.
- Comfortable and flexible with frequent change.
Estimated salary range: $90.00 - $95.00 per completed assessment, depending on county. Benefits include mileage reimbursement and 401(k) with limited voluntary benefits. Travel required to meet patients where they live. Cleveland, OH 44111
