Job Title: Patient Assessment Coordinator

CommuniCare Health Services Corporate is seeking a skilled Patient Assessment Coordinator to identify patients for potential admission into our facilities and maintain positive relationships with current referral sources.

The ideal candidate will have an RN or LPN license, a degree in Social Work, or other medical credentials. The Patient Assessment Coordinator will facilitate admissions by coordinating transmission of clinical and benefit information from referral sources back to the SNF admissions teams, administrations, and third party payers.

Key Responsibilities:
  • Performing clinical assessments
  • Resource to the Liaisons

The Patient Assessment Coordinator achieves these objectives by providing excellent, responsive customer service and accurate, timely, and comprehensive clinical assessments and by proactively working with all departments in the facility.

Requirements:
  • Preferred: Current, valid and active license to practice as a Registered Nurse or Licensed Practical Nurse in the state of Ohio, OR Social Work Degree, OR other medical credential.
  • Current, valid and active driver's license required.
  • Three (3) years treatment experience in a Long Term Care setting preferred.
  • Previous clinical screening experience preferred.
  • Recent healthcare experience in Long Term Care preferred.
  • Knowledge of local medical community preferred.
  • Ability to collect, organize, evaluate and communicate professionally and assertively, both verbally and in writing, pertinent clinical information with medical and administrative staffs, patients and families.
  • Strong customer service skills and commitment to service excellence.
  • Outgoing, energetic personality
  • Ability to project a professional image.
  • Knowledge of regulatory standards and compliance requirements.
  • Strong organizational, prioritizing and analytical skills.
  • Ability to make independent decisions when circumstances warrant.
  • Working knowledge of computer and software applications used in job functions.
