Nurse Care Coordinator Job Summary:

We are seeking an experienced Registered Nurse to join our Progressive Care Unit as a Nurse Care Coordinator. In this role, you will provide high-quality nursing care to patients with complex medical needs, working closely with our interdisciplinary team to ensure safe and effective care delivery.

Key Responsibilities:

  • Develop and implement individualized care plans for patients with complex medical needs.
  • Collaborate with healthcare team providers to ensure safe and effective care delivery.
  • Provide ongoing education and support to patients and families throughout the care process.
  • Maintain accurate records and reports on patient progress and outcomes.

Salary:$65,000 - $85,000 per year

Requirements:

  • Associate's Degree in Nursing (BSN degree required for ADN prepared nurses hired after February 24, 2017)
  • Registered Nurse license in the State of Kansas or compact license
  • BLS Certification
  • 6 months to 2 years' experience as a Clinical Nurse
