Job Description
  • The Ambulatory Clinical Nurse Specialist will be responsible for managing patient flow, triage, and direct patient care.
  • This role requires excellent communication and interpersonal skills, with the ability to work effectively with front desk staff, nursing staff, physicians, and clinic managers.
  • The successful candidate will have experience in developing expected patient outcomes, defining appropriate resource utilization, and working with providers to set patient goals.
Key Responsibilities
  1. Manage the care of the patient population by developing expected patient outcomes and defining appropriate resource utilization.
  2. Provide nursing support/coordination to the practice to optimize patient outcomes and improve patients' quality of life through clinical and educational programs.
  3. Care for high-risk patients with multiple co-morbidities or high risk for readmission to a hospital setting, including management of patients with multiple co-morbidities or high risk for readmission.
