About the Job
The Ambulatory Care Coordinator will play a vital role in ensuring seamless patient care at The University of Kansas Health System. This position is responsible for managing patient flow, triage, and direct patient care. As a liaison between front desk staff, nursing staff, physicians, and clinic manager, you will be the primary point of contact for patients and families, providing exceptional service and support throughout their healthcare journey.

Key Responsibilities

  • Develop and implement effective patient care plans to optimize health outcomes and quality of life
  • Collaborate with healthcare teams to coordinate continuity of patient care, including hospital admission, discharge, and ER visits
  • Manage high-risk patient care, including patients with multiple co-morbidities or high risk for readmission
  • Educate patients and families about healthcare needs, safety issues, and benefits through clinical and educational programs

Requirements
This role requires a High School Graduate with a Bachelor Degree in Nursing and 2+ years of RN experience. A multi-state Registered Nurse (RN) license is required, with proof of application for a multi-state RN license if applicable. Basic Life Support Heartsaver certification is also necessary. We offer a competitive salary of $75,000 - $95,000 per year, depending on experience.
