The role of a Foster Care Coordinator is essential in delivering high-quality care services to children and youth in foster care. This position requires a professional with expertise in trauma-informed approaches, evidence-based practices, and population management.

This is a field-based role located in Reno, Butler, and Sedgwick Counties, KS, and surrounding areas. As a Foster Care Coordinator, you will work closely with multidisciplinary teams to assess, plan, and implement care strategies tailored to individual needs. Your primary responsibilities will include:

  • Conducting thorough assessments and developing personalized care plans that address reunification, permanency planning, and transition-age youth needs.
  • Collaborating with healthcare providers, social workers, and other stakeholders to ensure timely referrals and coordination of psychosocial, behavioral, and medical services.
  • Serving as a single point of contact for families, caregivers, and service providers to facilitate communication and care coordination.
  • Advocating for children, youth, and families to ensure their voices are represented and supported by the healthcare team.

You'll be responsible for providing education on trauma-informed care, disease-specific interventions, and navigating complex systems like child welfare and juvenile justice. Additionally, you'll coordinate with educational advocates, behavioral health services, and state initiatives like the Family First Prevention and Services Act.

To succeed in this role, you'll need:

  • A Master's degree in social work, nursing, psychology, or a related human services area.
  • 2+ years of experience working within the community health setting, preferably in a healthcare role.
  • 1+ years of experience working with multidimensional children and youth.
  • Proficiency in MS Office applications, including Word, Excel, and Outlook.
  • Ability to travel within assigned regions to visit Medicaid members in foster homes and community settings.
