At Bear Mountain Healthcare, we are seeking a highly skilled Assessment Coordinator to join our team. As a key member of our clinical team, you will play a crucial role in maximizing facility reimbursement by working directly with our clinical team to ensure timely and accurate completion of UDAs (Uniform Data Assessments).

Key responsibilities include:

  • Generating UDAs prior to MDS ARD (with specific emphasis on GG and monitoring for timely completion)
  • Ensuring all UDAs are filled out and locked
  • Completing UDAs in a timely manner
  • Monitoring compliance with current and upcoming UDAs

As an Assessment Coordinator, you will work closely with our MDS Coordinator to ensure that MDS assessments have supporting documentation for what is coded on them. You will also be responsible for communicating with administrative and nursing leadership if UDAs are not being filled out and missing.

Requirements include a license as a nurse in the state of MA and the ability to work independently with minimal supervision.
