Clinical Care Coordinator

We're looking for a detail-oriented Clinical Care Coordinator to oversee the coordination of patient care across various settings. This is a remote position based in Michigan, with a 10-month duration.

The salary for this role is $40.00 per hour on a W2 basis.

In this role, you'll be responsible for ensuring the continuity of patient care by coordinating with healthcare professionals and organizations. You'll need to possess excellent communication and organizational skills, as well as a strong understanding of clinical practices and healthcare delivery processes.

Responsibilities:

  • Coordinate patient care across multiple settings.
  • Collaborate with healthcare providers to ensure seamless transitions of care.
  • Develop and maintain accurate records of patient care activities.

Requirements:

  • Certified Registered Nurse with an unrestricted Michigan RN license.
  • Two to four years of clinical experience, including acute patient care and discharge planning.
  • Knowledge of healthcare policies and procedures.

Preferred Qualifications:

  • Experience working in a managed care environment.
