Job Summary

We are seeking an experienced Registered Nurse to join our Case Management team as a Clinical Care Coordinator. In this role, you will be responsible for coordinating and overseeing patient care throughout their hospital stay, ensuring efficient and effective care while promoting positive outcomes.

Responsibilities:

  • Patient Assessment and Care Planning: Assess patient needs and develop comprehensive care plans in collaboration with physicians, social workers, and other healthcare professionals.
  • Care Coordination: Coordinate patient care across multiple departments, ensuring appropriate resources and interventions are provided.
  • Progress Monitoring: Monitor patient progress and adjust care plans as necessary.
  • Patient Education: Educate patients and families on treatment options, discharge planning, and community resources.
  • Discharge Planning: Ensure timely discharge by arranging post-hospital care and follow-up appointments.
  • Clinical Documentation: Document patient progress, interventions, and discharge plans accurately in the electronic health record.
  • Advocacy: Advocate for patients and facilitate the delivery of cost-effective care while maintaining quality standards.
