Job Description:

We are seeking a skilled and compassionate Clinical Social Worker to join our multidisciplinary team. In this role, you will provide direct patient care services, conduct assessments and develop treatment plans, and collaborate with healthcare professionals to ensure effective care coordination.

Duties and Responsibilities:
  • Provide comprehensive psychosocial assessments to identify patient strengths, challenges, and support systems.
  • Develop and implement individualized treatment plans that address patient needs and promote optimal health outcomes.
  • Collaborate with healthcare teams to ensure seamless care coordination and effective communication.
  • Provide education and support to patients and families on various topics, including coping skills, stress management, and resources available in the community.
  • Maintain accurate records and documentation of patient interactions and progress notes.

Required Skills and Qualifications:
  • Master's degree in Social Work (MSW) or related field required.
  • Current licensure as a Master's level social worker (LMSW) or Licensed Professional Counselor (LPC) preferred.
  • Minimum 1 year of experience in clinical social work settings, preferably in a hospital or healthcare setting.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations and interdisciplinary teams.
  • Able to maintain confidentiality and handle sensitive information discreetly.

Salary Range: $65,000 - $85,000 per year.
