Job Summary:
Retail healthcare providers are seeking a highly skilled Clinical Case Manager to facilitate seamless patient transitions between care levels. This role requires strong clinical judgment, excellent communication skills, and a deep understanding of healthcare systems. The ideal candidate will collaborate closely with patients, families, healthcare providers, and community resources to optimize patient outcomes and promote efficient resource utilization.

This position plays a pivotal role in coordinating comprehensive patient care, ensuring timely and appropriate interventions, including specialty consultations and referrals. Responsibilities include conducting comprehensive patient assessments, developing personalized care plans, and monitoring patient progress to identify potential barriers to care. Proactive strategies will be implemented to address these challenges and facilitate patient education and support.

A key component of this role is discharge planning, where the Clinical Case Manager will develop and implement comprehensive discharge plans that address the patient's physical, social, and financial needs. Collaboration with community resources and post-acute care providers ensures a smooth transition of care, advocating for patients' rights and access to necessary services and support.

The Clinical Case Manager will analyze patient outcomes, identify opportunities for improvement in care processes, and participate in quality improvement initiatives. They will stay updated on relevant regulations, guidelines, and industry trends to ensure compliance and optimize care delivery. Effective collaboration with diverse teams, including physicians, nurses, social workers, therapists, and other healthcare professionals, is essential for delivering coordinated care.

In addition to clinical expertise, the ideal candidate will possess strong organizational and time management skills, proficiency in electronic health records (EHR), and knowledge of case management principles and practices. Certification in Case Management (CCM) or Accredited Case Manager (ACM) is required within one year of hire. A Registered Nurse (RN) with active licensure and a valid State of Michigan license is essential.
