Clinical Team Manager

Key Responsibilities:
- Supervise and support direct care team members, ensuring seamless patient care delivery.
- Develop and implement patient care plans, focusing on clinical and administrative outcomes.
- Provide constructive feedback and coaching to team members, promoting growth and development.
- Collaborate with patients, families, and healthcare teams to ensure comprehensive care.
- Manage patient expectations, advocate for their needs, and promote a safe and cost-effective care environment.
- Stay up-to-date with industry developments, best practices, and regulatory requirements.
- Communicate effectively with team members, patients, and families, ensuring clear and timely information exchange.
Requirements:
- Associate's degree in a Nursing-related field.
- Current State License as a Registered Nurse.
- 2+ years of home care experience as a Registered Nurse in a Home Health or Hospice environment.
- 1+ year of supervisory experience.
- Advanced nursing skills, including patient care episode management, plan of care changes, clinical evaluations, and recommendations.
- Ability to develop trusted relationships, leverage personal networks, and communicate effectively across levels.
- Valid driver's license and reliable transportation.
