Key Responsibilities:

The Clinical Team Manager is responsible for the coordination of care during the home health benefit period, staff engagement and communication, safety, and training of staff. This includes:

  • Supervising and overseeing patient care plans
  • Ensuring clinical and administrative outcomes are met
  • Providing professional and supportive services to clients in a safe and cost-effective manner
  • Coordinating care during the home health benefit period
  • Engaging and communicating with staff to ensure a collaborative environment
  • Ensuring safety protocols are in place
  • Training staff to meet the needs of our patients

Requirements:

To be successful in this role, you will need:

  • An Associate's degree in a Nursing-related field
  • A current State License as a Registered Nurse
  • 2 years of home care experience as a Registered Nurse in a Home Health or Hospice environment
  • 1 year of supervisory experience
  • Advanced nursing skills, including the ability to manage patient care episodes, changes to plan of care, clinical evaluations, and recommendations
  • The ability to provide constructive feedback and develop clinical quality, core competencies, and productivity with direct reports
  • The ability to seek patient feedback, advocate for patients, understand patient needs, and manage patient expectations
  • Quickly become proficient in enterprise applications (Homecare Homebase, Workday, etc.) and intermediate knowledge of Microsoft Office
  • The ability to convey information and opinions, listen to others, and check for understanding
  • The ability to develop trusted relationships and networks, leverage personal networks, and relate across supervisory and leadership levels
  • A valid driver's license and reliable transportation to perform job tasks
