Hospice LPN Clinical Coordinator

As a Licensed Practical Nurse Clinical Coordinator (LPN CC) in our Hospice team, you will be instrumental in ensuring seamless patient care by supporting Clinical Team Managers (CTMs) and coordinating the care journey for our patients.
Key Responsibilities- Maintain accuracy in documentation through Homecare Homebase (HCHB) to ensure all workflow standards are met.
- Serve as the main connection between patients, CTMs, medical staff, and facilities, enhancing communication and care coordination.
- Assess and respond to patient needs, advocating on their behalf to ensure the highest quality of care and compassion.
- Use sound judgment to make decisions, manage complex situations, and escalate issues when necessary.
- Address and resolve complaints effectively to ensure positive outcomes for patients and staff.
- Foster a collaborative work environment, offering guidance and support to help your team thrive.
- Licensed Practical Nurse (LPN): Graduate of an accredited nursing school with an active LPN certification.
- Relevant Experience: 1+ year of practical experience, ideally in Home Health or Hospice care.
- Strong Communication Skills: Ability to communicate clearly, actively listen, and adapt messaging as needed.
- Time Management Skills: Organized, efficient, and able to handle multiple priorities.
- Tech-Savvy: Proficient with systems like Homecare Homebase, Workday, and Microsoft Office.
- Patient-Centered Focus: Skilled in understanding and addressing patient needs, with a commitment to advocating on their behalf.
- Bilingual (Preferred): Proficiency in Spanish is a plus.
- Valid Driver's License & Reliable Transportation: Required for travel in support of field duties.
