Care Coordinator for Behavioral Health

At Delta-T Group, Inc., we provide cost-effective and reliable referrals to independent contractors in the social services and behavioral health sectors. As a Care Coordinator, you will have the opportunity to work with adults with psychiatric conditions and promote their integration into the community.

About the Role

In this position, you will work with each individual to build life skills in areas such as daily living, medication administration, symptom reduction, health and wellbeing, employment, school, socialization, and recreation. You will also be responsible for transporting individuals using your operational and insured vehicle.

Requirements
  • A High School Diploma or equivalent is required; a Bachelor's degree in human services, psychology, or a related field is preferred.
  • 1-2 years of experience working with adults with psychiatric conditions and/or homeless populations.
  • You must have access to an operational and insured vehicle and be willing to use it to transport individuals.
  • Preference is given to bilingual/bicultural applicants and those with lived experience of psychiatric conditions.
  • All candidates for this position must pass a state criminal background check, driving record, reference, educational, and professional licensure check.
