Care Coordinator RN IDD Services

Sevita is seeking a skilled and compassionate Registered Nurse to join our team as a Care Coordinator in IDD Services. This hybrid position offers the perfect blend of flexibility and fulfillment, with a part-time schedule of 20 hours per week.
About the RoleAs a Care Coordinator RN, you will play a vital role in ensuring the seamless coordination of client discharges, conducting thorough intake assessments for new clients, and managing medication orders. Your strong nursing background and excellent communication skills will enable you to effectively collaborate with staff and families to provide exceptional care.
Responsibilities- Coordinate client discharges, including developing discharge plans and arranging necessary follow-up care
- Conduct intake assessments for new clients, gathering comprehensive information on their medical history, treatment needs, and support systems
- Manage medication orders, including auditing in-home medications and medication administration records, and providing education to caregivers as needed
- Collaborate with interdisciplinary teams to ensure high-quality, patient-centered care
- Valid state nursing license as an RN
- Minimum 1 year of relevant experience
- Valid Driver's License - 1+ year and vehicle
- Self-motivated and detail-oriented with ability to multi-task
- Excellent communication and analytical skills
- Flexible start times and no on-call responsibilities
