Job SummaryWe are seeking a skilled Certified Nursing Assistant (CNA) to join our Home Health Care Partners PACE team. As a CNA, you will play a vital role in delivering exceptional patient care and supporting our patients in achieving their goals.
Key Responsibilities- Provide direct patient care, including assistance with activities of daily living, mobility, and elimination.
- Obtain and record patient information, including vital signs, height and weight, blood pressure, oxygen saturation, intake and output, calorie counts, and other relevant data.
- Assist patients with transfers, ambulation, and use of walkers, crutches, canes, and wheelchairs.
- Collect and process specimens for diagnostic or waived tests.
- Engage in evidence-based best practices that enhance the patient experience, including SAFETY rounding, communication fundamentals/AIDET, Teach-Back, Narrating Care, etc.
- Basic Cardiac Life Support certification within 30 days of employment start date.
- Certified Nursing Assistant (CNA) certification.
- Valid driver's license.
- High school graduate or equivalent.
