Clinical Liaison Specialist

The Clinical Liaison position is a key role in our organization, responsible for building and maintaining strong relationships with healthcare providers, patients, and families to ensure seamless care transitions and optimal patient outcomes.
Key Responsibilities- Develop and maintain relationships with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services.
- Facilitate the coordination of services and ensure proper documentation for compliance with agency and statutory guidelines.
- Work collaboratively with the care team to ensure a smooth transition of patients from acute care facilities to their homes or next location of residence.
- Provide education and support to patients and families on available services and resources.
- Collaborate with the marketing team to promote our services and programs to healthcare providers and patients.
- Education sufficient for clinical licensure for discipline (e.g., LPN, LVN, Registered Dietitian) or Bachelor's Degree in Social Work.
- Current state professional license or certification appropriate to discipline.
- 3 years of experience in clinical discipline.
- Master's Degree in Social Work.
- Bachelor's Degree in Applicable clinical discipline (e.g., Respiratory Therapy).
- Liaison, marketing, or sales experience.
- Healthcare industry experience.
- Documented prior successful sales account medical management experience.
- Epic EMR experience.
