Clinical Liaison Specialist

We are seeking a skilled Clinical Liaison to join our team at Providence Health & Services. As a Clinical Liaison, you will play a critical role in creating and maintaining positive customer relationships to increase referrals for Home Health, Hospice, and other services.
Key Responsibilities- Assess appropriateness for hospice and home health services and facilitate the coordination of services
- Ensure proper documentation for compliance with agency and statutory guidelines
- Develop and maintain relationships with discharge planners, facility staff, physicians, and individuals
- Collaborate with cross-functional teams to ensure seamless transitions of care
- Education sufficient for clinical licensure for discipline (LPN/LVN) or Bachelor's Degree in Social Work
- Current state professional license or certification appropriate to discipline
- 3 years of experience in clinical discipline
- Master's Degree in Social Work
- Bachelor's Degree in Applicable clinical discipline (e.g., Respiratory Therapy)
- Liaison, marketing or sales experience
- Healthcare industry experience
