Clinical Liaison Specialist

The Clinical Liaison position is a key role in creating and maintaining positive customer relationships to increase referrals for Home Health, Hospice, Home Infusion, PACE, Transitional Care Unit, and other services within assigned accounts.
Key Responsibilities- Work with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services.
- Facilitate the coordination of services and ensure there is appropriate documentation for compliance with agency and statutory guidelines.
- Manage the transition of patients from acute care facilities to their homes or next locations of residence.
- Education sufficient for clinical licensure for the discipline (e.g., LPN, LVN, Registered Dietitian) or a Bachelor's Degree in Social Work.
- Current state professional license or certification appropriate to the discipline upon hire.
- 3 years of experience in the clinical discipline.
- Master's Degree in Social Work.
- Bachelor's Degree in an applicable clinical discipline (e.g., Respiratory Therapy).
- Liaison, marketing, or sales experience.
- Healthcare industry experience.
- Documented prior successful sales account medical management experience.
- Epic EMR experience.
