Clinical Liaison Specialist

We are seeking a skilled Clinical Liaison to join our team at Providence Health & Services. As a Clinical Liaison, you will play a critical role in creating and maintaining positive customer relationships to increase referrals for Home Health, Hospice, and other services.
Key Responsibilities- Assess appropriateness for hospice and home health services and facilitate the coordination of services.
- Work with discharge planners, facility staff, physicians, and individuals to ensure seamless transitions of patients from acute care facilities to their homes or next locations of residence.
- Develop and maintain strong relationships with healthcare professionals to promote our services and improve patient outcomes.
- Ensure compliance with agency and statutory guidelines through accurate documentation and coordination of services.
- Education sufficient for clinical licensure for discipline (LPN/LVN) or Bachelor's Degree in Social Work.
- Current state professional license or certification appropriate to discipline.
- 3 years of experience in clinical discipline.
- Master's Degree in Social Work.
- Bachelor's Degree in Applicable clinical discipline (e.g., Respiratory Therapy).
- Liaison, marketing, or sales experience.
- Healthcare industry experience.
- Documented prior successful sales account medical management experience.
- Epic EMR experience.
