Clinical Liaison Specialist

We are seeking a skilled Clinical Liaison to join our team at Providence Service. As a Clinical Liaison, you will play a critical role in creating and maintaining positive customer relationships to increase referrals for our services.
Key Responsibilities- Assess appropriateness for hospice and home health services
- Facilitate the coordination of services
- Ensure proper documentation for compliance with agency and statutory guidelines
- Work with discharge planners, facility staff, physicians, and individuals to transition patients from acute care facilities to their homes or next locations of residence
- Education sufficient for clinical licensure (e.g., LPN, LVN, Registered Dietitian) or Bachelor's Degree in Social Work
- Current state professional license or certification appropriate to discipline
- 3 years of experience in clinical discipline
- Master's Degree in Social Work
- Bachelor's Degree in applicable clinical discipline (e.g., Respiratory Therapy)
- Liaison, marketing, or sales experience
- Healthcare industry experience
- Documented prior successful sales account medical management experience
- Epic EMR experience
