Key Responsibilities:
• Provide personal care services to patients based on assigned tasks and respond promptly to their needs.
• Communicate effectively with patients and their families while maintaining confidentiality and respect.
• Ensure patient safety by using equipment properly and adhering to safety protocols.
• Assist with daily living activities, including bathing, grooming, and dressing as required.
• Prepare nutritious meals and assist with feeding as needed.
• Monitor and record vital signs as per care plans and report any significant changes to the Case Manager.
• Support patients with mobility and exercise routines as directed by healthcare professionals.
• Maintain a clean and organized living environment for patients.
• Participate in quality improvement initiatives and adhere to organizational standards.

Qualifications:
• Completion of state-required training and certification.
• Minimum of 75 hours of classroom and supervised practical training.
• At least six months of experience in personal care services.
• Must be at least 18 years old and able to follow written instructions.
• Understanding of hospice care principles and comfort in providing care to terminally ill patients.
• Current CPR certification and satisfactory references from previous employers.
• Ability to work independently and maintain a calm demeanor in challenging situations.
• Demonstrated patience, tact, and good personal hygiene.
