**Position Summary**


The Hospice Registered Nurse is responsible for planning, organizing, and directing hospice care, drawing on extensive nursing experience with a focus on community health education.

This professional role leverages community resources to develop and implement services tailored to the unique needs of patients and their families within their homes and communities.

**Key Responsibilities**

  • Conducts initial, comprehensive, and ongoing assessments of patients and families to identify hospice requirements.
  • Performs thorough physical assessments and compiles medical histories of current and past illnesses.
  • Delivers professional nursing care by applying all aspects of the nursing process.
  • Evaluates patient status through:
  • Formulating and initiating a care plan.
  • Regularly reassessing patient and family/caregiver needs.
  • Adjusting the care plan as necessary based on patient status and requirements.
  • Utilizing health assessment data to establish nursing diagnoses.
  • Creating a care plan that sets goals based on nursing diagnoses and includes palliative nursing interventions.
  • Involving patients and families in the planning process.
  • Initiating appropriate preventive and rehabilitative nursing actions.
  • Administering medications and treatments as prescribed by the physician.
  • Providing counseling to patients and families to address nursing and related needs.
  • Offering health care instructions to patients as per assessment and care plan.
  • Assisting patients with daily living activities and promoting self-sufficiency and comfort care.
  • Acting as Case Manager when designated by the Clinical Supervisor, coordinating care for assigned patients.
  • Documenting and maintaining accurate clinical notes regarding patient conditions and care provided.
  • Recording changes and outcomes in pain/symptom management as appropriate.
  • Communicating with physicians regarding patient needs and reporting any changes in conditions.
  • Collaborating with community health professionals to coordinate care plans.
  • Educating patients and families on self-care techniques as suitable.
  • Providing medication, dietary, and other instructions as directed by the physician while recognizing opportunities for health counseling.
  • Collaborating with the interdisciplinary team.
  • Ensuring a safe environment for patients.
  • Supporting continuity of care for patients and families.
  • Working alongside families and the hospice interdisciplinary team to address emotional needs.
  • Participating in interdisciplinary meetings.
  • Engaging in on-call duties as outlined by policy.
  • Ensuring availability of necessary equipment and services.
  • Supervising ancillary staff and delegating responsibilities as needed.
  • Committing to personal growth and development by enhancing professional knowledge and skills through ongoing education.
  • Fulfilling responsibilities for accepted case assignments.
  • Actively engaging in quality assessment and performance improvement initiatives.
**Qualifications**

  • Graduate from an accredited nursing program.
  • Current Registered Nurse licensure and American Heart Association CPR certification required; CPR training may be provided upon hire.
  • Bachelor's degree preferred, with at least one year of hospice experience preferred.
  • Management experience is not mandatory; responsible for supervising hospice aides.
  • Strong observational, verbal, and written communication skills, along with problem-solving and basic math skills; nursing competencies as per checklist.
