Key Responsibilities:
- Deliver personal care services based on patient assignments and respond promptly to their requests.
- Communicate effectively with patients and families, ensuring ethical and respectful care while maintaining confidentiality.
- Ensure patient safety by using equipment correctly and maintaining a safe environment.
- Provide personal care, including bathing, oral hygiene, and changing bed linens as needed.
- Monitor and record vital signs such as temperature, pulse, respiration, and blood pressure as per care plans.
- Observe and care for patients' skin to prevent tissue breakdown.
- Assist with mobility and exercise as directed by nursing staff.
- Support light laundry tasks as assigned.
- Perform range of motion exercises and other simple procedures as directed.
- Maintain cleanliness and orderliness in the patient's living area.
- Adhere to documentation and care standards set by the organization.
- Participate in quality improvement initiatives and training programs.
Qualifications:
- Completion of a minimum of seventy-five hours of classroom and supervised practical training.
- At least six months of experience in providing personal care.
- Ability to read, follow written instructions, and document care provided.
- Understanding of hospice philosophy and comfort in providing specialized care to terminally ill patients.
- Capability to work independently with minimal supervision.
