Key Responsibilities:

  • Deliver personal care services based on patient assignments and respond promptly to their requests.
  • Communicate effectively with patients and families, ensuring ethical and respectful care while maintaining confidentiality.
  • Ensure patient safety by using equipment correctly and maintaining a safe environment.
  • Provide personal care, including bathing, oral hygiene, and changing bed linens as needed.
  • Monitor and record vital signs such as temperature, pulse, respiration, and blood pressure as per care plans.
  • Observe and care for patients' skin to prevent tissue breakdown.
  • Assist with mobility and exercise as directed by nursing staff.
  • Support light laundry tasks as assigned.
  • Perform range of motion exercises and other simple procedures as directed.
  • Maintain cleanliness and orderliness in the patient's living area.
  • Adhere to documentation and care standards set by the organization.
  • Participate in quality improvement initiatives and training programs.

Qualifications:

  • Completion of a minimum of seventy-five hours of classroom and supervised practical training.
  • At least six months of experience in providing personal care.
  • Ability to read, follow written instructions, and document care provided.
  • Understanding of hospice philosophy and comfort in providing specialized care to terminally ill patients.
  • Capability to work independently with minimal supervision.
