Key Responsibilities

  • Conduct initial and ongoing thorough assessments of patients and their families
  • Develop and initiate patient care plans, adjusting as necessary
  • Utilize health assessment data to formulate nursing diagnoses
  • Administer prescribed medications and treatments according to the care plan
  • Provide counseling to patients and families to address nursing and related needs
  • Maintain accurate and detailed clinical documentation
  • Communicate effectively with physicians and participate in interdisciplinary meetings to address the medical and emotional needs of patients and their caregivers
  • Ensure adherence to all applicable state and federal regulations
  • Perform additional duties as assigned
Qualifications

  • Graduate from an accredited nursing program with current licensure in the state of practice
  • Minimum of 1 year of nursing experience required; 3+ years preferred
  • Experience in hospice care or a similar environment is preferred
  • Understanding of the hospice care philosophy
  • Dedication to clinical excellence and thorough documentation

USD $95,000.00 - $100,000.00 / Year
