Position Overview:
As a Clinical Social Services Specialist, you will have the opportunity to collaborate closely with our Director of Clinical Services and other dedicated team members, including physicians, chaplains, and therapists. Your role will be crucial in delivering essential social services to patients and their families, acting as a trusted problem solver.

Key Responsibilities:
- Provide short-term individual counseling and crisis intervention.
- Assist in the preparation of advance directives and offer guidance on funeral planning matters.
- Help patients and families navigate fiscal, legal, and healthcare decisions.
- Identify and connect families with support systems to alleviate stress and aid in coping with end-of-life care.
- Assess community resources and evaluate the patient/family's ability to access them.
- Perform additional related duties as required.

Desired Skills and Qualifications:
