Position Overview:

As a Personal Support Specialist, you will provide essential supervision and guidance to individuals, promoting their personal growth and development across various supported living arrangements and recreational activities.

Responsibilities:
  • Manage a designated caseload effectively.
  • Maintain comprehensive and accurate records for individuals and report on their progress.
  • Facilitate Individual Program and semi-annual meetings.
  • Ensure compliance with medical, dietary, and psychological recommendations as outlined in each individual's Plan of Service.
  • Collaborate with an interdisciplinary team, including family members and healthcare professionals.
  • Receive guidance from the Director of Residential Fee for Service Programs.
  • Document program data, medical records, and case notes accurately and promptly.
  • Complete billing documentation in a timely manner.
  • Actively engage as a member of the Kennedy Center team.
  • Assist in developing and implementing Individual Program Plans.
  • Create suitable recreational activities tailored to each individual.
  • Maintain communication with families and guardians as necessary.
  • Ensure the protection of each individual's rights in accordance with agency policies.
  • Transport individuals for medical and recreational appointments as needed.
  • Perform household tasks as assigned.
  • Complete required training and in-services as per agency guidelines.
  • Schedule medical appointments for individuals as assigned.
  • Keep electronic medical records updated, including appointments and medications.
  • Communicate with healthcare coordinators as required.
  • Perform other related duties as assigned.
Qualifications:
  • High School Diploma.
  • Valid Driver's License and ability to obtain a State of CT Public Service License.
  • Maintain a satisfactory driving record.
  • Possess a vehicle on-site during shifts for transportation needs.
  • Ability to work in various settings and weather conditions.
  • Maintain certifications in PMT and CPR, and be eligible for DDS medication certification.
  • Proficient in Microsoft Office and related software.
  • Flexible schedule availability, including early mornings, evenings, weekends, and holidays.
  • Ability to work independently with sound judgment and initiative.
  • Strong verbal and written communication skills.
  • Access to all areas of residential facilities.
Physical Requirements:
  • Provide hands-on personal care, including assistance with daily living activities.
  • Ability to assist individuals requiring personal hygiene care.
  • Physical endurance to implement crisis intervention techniques.
  • Capability to lift and move individuals as necessary.
  • Ability to climb stairs and lift weights up to 50 lbs. occasionally.
