Job Purpose:

To provide appropriate oversight and guidance to individuals in our care, promoting their personal growth and development across various supported living environments and recreational and therapeutic programs.

Key Responsibilities:
  • Serve as case manager for assigned individuals.
  • Maintain precise records and documentation, submitting progress reports as required.
  • Facilitate and attend individual planning and semi-annual meetings.
  • Ensure adherence to medical, dental, dietary, psychological, and other habilitative service recommendations as outlined in the individual's Plan of Service.
  • Collaborate with an interdisciplinary team, including family members, registered nurses, and behavior specialists.
  • Receive direct supervision from the Director of Residential Fee for Service Programs.
  • Accurately document and maintain program data, medical records, financial records, and case notes related to recreational activities.
  • Complete necessary billing documentation promptly.
  • Actively engage as a team member within The Kennedy Center and interdisciplinary teams.
  • Assist in the development and implementation of Individual Program Plans.
  • Create suitable recreational activities tailored to each individual.
  • Maintain communication with families and/or guardians as assigned.
  • Ensure the protection of each individual's rights in accordance with agency policies and procedures.
  • Transport individuals for medical appointments, recreational activities, and other necessary engagements.
  • Perform various household tasks as assigned.
  • Complete required in-service training annually and other job-related training as specified by agency policies.
  • Schedule medical appointments for individuals as needed.
  • Keep electronic medical records current, including appointments, medications, and follow-up needs.
  • Communicate with healthcare coordinators as assigned.
  • Other related duties as assigned.
Qualifications:
  • High School Diploma.
  • Valid Driver's License and ability to obtain a State of CT Public Service License.
  • Maintain a satisfactory driving record.
  • Possess a vehicle on-site during shifts to transport individuals as needed.
  • Ability to work in various settings and weather conditions.
  • Maintain PMT, CPR certification, and eligibility for DDS medication certification.
  • Proficient in Microsoft Office, Therap, and internet navigation.
  • Flexible schedule availability, including early mornings, evenings, weekends, and holidays.
  • Ability to work independently, exercise good judgment, and demonstrate initiative.
  • Effective communication skills in both oral and written forms.
  • Access all areas of residential facilities.
Physical Requirements:
  • Provide hands-on personal care, including toileting, feeding, and daily living activities.
  • Ability to assist individuals requiring personal hygiene care.
  • Physical endurance to implement crisis intervention techniques, including restraint if necessary.
  • Provide physical assistance, including lifting and mobility support.
  • Ability to climb stairs and lift weights up to 10 lbs. regularly and 50 lbs. occasionally.
