This position is focused on achieving the best possible outcomes for patients within the acute care environment. The case management approach ensures that patient needs are effectively matched with appropriate resources to fulfill quality, clinical, and financial objectives.

Qualifications

- Current registration as a Registered Professional Nurse in New York State is mandatory.
- A Bachelor of Science in Nursing is required within five years of employment in this role.
- A minimum of 2 years of experience in acute care nursing is essential.
- Preference will be given to candidates with 2 years of experience in case management, home care, or utilization review.
- Familiarity with Interqual Criteria and related software systems is advantageous.
- Proficiency in Microsoft Office Suite and Outlook is required.
- Experience with healthcare-related computer applications is necessary.
- Certification as a Case Manager (CCM) is preferred.
- Basic Life Support (BLS) certification is required upon hire and must be maintained thereafter.

Compensation

This position is eligible for a sign-on bonus. The salary range for this role is $43.77 to $56.01 per hour, with final compensation determined by factors such as experience, qualifications, internal equity, and relevant collective bargaining agreements.

Work Schedule

Bi-weekly hours are set at 75, with scheduled work hours varying between 8:00 AM to 5:00 PM. Weekend and holiday scheduling may be required.
